Insurance/Payment

At this time, we participate with Carefirst BCBS, Cigna (Evernorth), Optum/United Health Care, Anthem PPO, Optima, Aetna, and Medicare.

Please always contact your insurance and request the authorization number for behavioral health, if needed, before making an appointment at the clinic.  The authorization number might be needed to schedule the first appointment.

Many insurance companies have out-of-network benefits and we will provide a statement with appropriate billing codes for patients to submit for reimbursement.  Please contact the insurance company or employee benefit plan to find out the services and limits of coverage the specific plan has.

Payment

All copay, coinsurance, and deductible amounts are due when the service is delivered.  We accept payment through credit, HSA, and debit at this time.  Sliding scale payment on selected clients is considered also.

Cancellation Policy: Please note that a $75 cancellation fee is charged for missed appointments or appointments cancelled with less than 48 hours notice (weekends and holidays are excluded from this 48 hour period.)

Also, there is a fee for the declined credit card and please update the information on the clinic database often.
