Setting up the intake appointment requires filling out online intake forms. Please follow the emailed instruction to create accounts on a secure messaging portal, https://secure.hushmail.com/mail/ ( the portal setup is explained here. ) AND on an Electronic Medical Record system. Please request a help on setting up the portal by calling the clinic, if you experience any technical difficulty.
